1. Our Commitment to Your Health Information Privacy

At Serene Meadows Senior Care Community LLC (doing business as Serene Meadows Living), we understand that medical and personal health information about our residents is private and confidential. We are mandated by federal law (Health Insurance Portability and Accountability Act of 1996, or HIPAA) and California Department of Social Services Title 22 regulations to protect the privacy of Protected Health Information (PHI).

This notice describes how medical information about residents may be used and disclosed, and how families and authorized representatives can access this information.

2. How We May Use and Disclose Protected Health Information (PHI)

• For Direct Clinical Care & Treatment: We may share health records with our Medical Director, registered nurses, visiting physical therapists, and outside primary care physicians to formulate and execute tailored care plans.
• For Emergency Situations: In urgent medical emergencies, paramedics and hospital emergency personnel will receive necessary health histories and current medication administration records (eMAR).
• With Designated Family Representatives: Information is shared only with individuals legally designated on the resident’s HIPAA Release of Information authorization and Durable Power of Attorney (DPOA) for Healthcare.
• As Required by State Licensure Law: We may disclose records to the California Department of Social Services (CDSS) or local ombudsman during official quality compliance surveys.

3. Resident Privacy Rights

Residents and their legal healthcare proxies possess the right to:

• Inspect and obtain a digital or paper copy of health and billing records.
• Request confidential communications through designated contact methods.
• Request corrections or amendments to incomplete or inaccurate health records.
• Receive an accounting of non-routine disclosures of health information.
• Revoke written authorization for information sharing at any time.

4. Website Data Collection & Security

When you use our website (e.g., scheduling a tour, utilizing the pricing estimator, or submitting an inquiry), we collect contact information (such as name, email, phone number, and care preferences) solely for the purpose of communicating with you regarding senior care services.

We never sell, rent, or trade family contact information to third-party senior placement agencies or advertising brokers. All submitted forms are protected with 256-bit SSL encryption.
